Private Practice Closure
NOTICE IS HEREBY GIVEN that Kurt LaRose, LCSW, Florida and Missouri Licensed Clinical Social Worker, FL License No SW9297 and MO License No 2020023042, formerly practicing as a sole proprietor online through TALKIFUWANT.COM, and formerly with offices in Tallahasseee, has permanently ceased the operation of his clinical social work practice due to medical reasons.

Clinical operations ended on MAY 3, 2026, following efforts during FEBRUARY, MARCH AND APRIL 2026 to determine whether the practice could remain in operation along with consult and reliance upon input from medical teams. Former clients of LaRose served while he practiced at the behavioral health unit with the Missouri hospital, if seeing this notice, were not treated by the private practice, and hospitial patients need to contact the hospital in Missouri for information and records requests.

All clinical records of the private practice are maintained in the HIPAA compliant secure server and will remain accessible for 7 years from the last date of service. Clients of the practice who wish to obtain electronic copies of their records, or to request that their records be transferred to another provider may submit a written request to LaRose, that includes client name, date of birth, current address and phone number, along with who the electronic records should be sent. Records can be sent via email with secure password encryption and can also be mailed using memory drives via fed ex or USPS, signature requested. Records requests can be sent to Kurt@TalkifUwant.com, or by USPS mail to: Kurt LaRose, 1701 E Harvest Circle, Perryville MO 63775-9330. Please provide your signature on the form you send. LaRose will verify all information prior to sending records.

This, or a similar notice, was first published in Tallahassee media on May 8, and on May 14 in Missouri media. The media ads will run 4 consecutive weeks.
